10/- only) Affidavit I, Shri________________________, son of ____________________________, r/o _________________________________ _______________________ solemnly affirm and declare and undertake as under: 1.
That I am competent to swear this affidavit on behalf
of M/s _____________________________, a company registered under the Companies Act,
1956 with registered corporate office at _______________________. The authorization letter issued by the
company to this effect in my favour is annexed hereto as Annexure-A. 2.
That I hereby furnish the bar charts of installation of the end-use plant (Power
plant/Iron & Steel plant/sponge iron plant/cement plant etc.,) and the development of the captive coal mine with
detailed schedules according to which I shall install the plant/extension of the plant and
develop the coal mine. These are annexed as
Annexure – B to this affidavit. 3.
That the entire coal produced from the captive
coal mining block allocated to the company and
the captive coal mine developed by the company shall
be used by the company in the end-use plant (power plant/iron
& plant/sponge iron plant/cement plant) for production of
________________/____________________/_______________for which the coal mining block has
been allocated to the company. 4.
That in case of slippage in the schedule of /between installation of the
end-use plant and the development of captive coal mine as provided in
the bar charts etc., the company shall forfeit the
captive coal mining block and the coal mining
block shall be de-allocated. Upon
de-allocation any and all rights in or over the said captive block shall stand
extinguished. 5. That the company shall have no claims to any costs incurred by it in the process. DEPONENT VERIFICATION Verified at New
